- Discuss with the client the requirements checklist, processing steps, and visa waiver.
- Obtain a signed visa waiver from the client and file it for record-keeping.
- Provide the client with a draft DS-160 application form to be completed at home and returned once accomplished, together with their requirements.
- Once the client returns the draft DS-160 form and requirements, review and scan all submitted documents and the application, noting any items that need correction. Ensure no field is left blank; instruct the client to write “N/A” for items that are not applicable or available.
- Collect full visa and processing fees. Inform the client that if the visa is approved, an additional ₱750 embassy courier fee will be payable upon passport release. Passport with visa pickup will be at the Cherieshed Travel Office.

PASSPORT APPLICATION & RENEWAL
STEPS IN PASSPORT PROCESSING
- Print the CHI Passport Forms
- Two (2) copies of Page 1-4 . print back to back.
- Print only the Special Power of Attorney (SPA) for clients with pickup requests and/or minors.
- Have the client fill out and sign all required forms.
- Access the DFA Appointment System and check available schedules, then confirm the appointment based on the client’s availability.
- Once confirmed, request payment either in cash or through online payment and issue only a BIR receipt for the assistance fee collected. The receipt shall reflect only the assistance fee and not the total payment. DFA official payment receipts will be sent directly to the client via email together with the appointment documents after appointment confirmation.
- Proceed with the passport application booking once payment and details are completed.
- Ask the client to forward the DFA appointment confirmation email to visa@cherieshed.com to facilitate printing of the appointment documents and schedule management.
- Print the appointment confirmation documents and photocopy all required supporting documents.
- Finally, prepare and arrange a complete folder containing all documents that the client must bring on the day of their DFA appointment.
NOTICE
- Appointment booked at any DFA Consular Office is non-transferable and cannot be rescheduled. If an applicant wishes to make any changes after obtaining their appointment, the existing appointment must first be cancelled. A new appointment can then be booked and paid.
- Payments can be made during the online appointment booking process or via the over-the-counter option through the DFA payment merchant partners. If the chosen mode of payment is through debit/credit card, cards or e-wallets should be readied prior to the registration. Only charges shown during the online booking process are to be paid. No other fee will be paid on-site, nor should applicants pay extra fees to obtain an appointment. (This is only applicable to applications submitted within the Philippines)
- If you have an urgent travel due to emergency and employment reasons, you are advised to choose a pick-up release of passport. Otherwise, applicants may avail of the online courier delivery service. Processing time for passports released via courier will add 10-15 working days. - FOREIGN REQUEST : Delivery destination is outside the Philippines; USD 20.30 per copy of birth, marriage and death certificates, and USD 25.30 per copy of Certificate of No Marriage Record (CENOMAR) and Certificate of No Death Record (CENODEATH) via PHLPost.
- GOVERNMENT AGENCY E-COPY REQUEST : Delivery destination is the address of the government agency; PHP 300.00 per copy of birth, marriage and death certificates, and PHP 360.00 per copy of Certificate of No Marriage Record (CENOMAR) and Certificate of No Death Record (CENODEATH).

2. PSA Serbilis uses the online payment facility of PesoPay. UNISYS PSASERBILIS will appear in the credit card statement or transaction record, and in the official receipt(s).
3. It is possible that the fee will be reflected in the requester credit card statement of account or transaction record before the document is received.
PAYMENT - REQUESTS FOR VIEWABLE ONLINE COPY
1. The viewable online copy of the PSA certificate is accessible for 60 days through an access code.
2. Viewable online requests can be paid at a PSA outlet only; PHP 130.00 for birth, marriage and death certificates, and PHP 185.00 for Certificate of No Marriage Record (CENOMAR) and Certificate of No Death Record (CENODEATH).
Fees are inclusive of processing and government taxes, including PHP 30.00 Documentary Stamp Tax (Sections 173 and 188 of the Tax Code, Revenue Memorandum Order 13-2008 and Revenue Regulations No. 4-2018) per copy issued.
3. Personal appearance at a PSA outlet is required. Viewable online requests not paid at a PSA outlet (paid online or through other payment channels) will not be processed. Payment must be made in person by the document owner (or nearest of kin of a deceased person) at any PSA CRS outlet. Payment by authorized representative is not allowed.
PAYMENT - REQUESTS FOR ELECTRONIC ENDORSEMENT
1. Generation of Electronic Endorsement Request Forms through PSA Serbilis is free of charge. The Electronic Endorsement Request Form should be printed and presented at the concerned Local Civil Registry Office.
2. Any fees to be collected by the Local Civil Registry Office shall be to the requester's/document owner's account.
PAYMENT - REQUESTS FOR PICK UP AT PSA OUTLET
1. PSA Serbilis accepts requests for pick up at any PSA outlet.
2. Requests for pick up at a PSA outlet can be paid either online or at the PSA outlet only; PHP 155.00 per copy for birth, marriage and death certificates, and PHP 210.00 per copy for Certificate of No Marriage Record (CENOMAR) and Certificate of No Death Record (CENODEATH).
Fees are inclusive of processing and government taxes, including PHP 30.00 Documentary Stamp Tax (Sections 173 and 188 of the Tax Code, Revenue Memorandum Order 13-2008 and Revenue Regulations No. 4-2018) per copy issued.
3. Philippine Statistics Authority uses the online payment facility of Maya. Philippine Statistics Authority will appear in the credit card statement or transaction record, and in the official receipt(s).
4. It is possible that the fee will be reflected in the requesters credit card statement of account or transaction record before the document is received.
5. Payment and/or pickup must be made in person by the document owner (or nearest of kin of a deceased person) at any PSA CRS outlet. Payment and/or pickup by authorized representative is not allowed.
REFUND AND CANCELLATION
After payment is made, requests cancellation can no longer be accepted. PSA Serbilis adopts a NO REFUND and NO CANCELLATION policy for all its services.
ORDER FULFILLMENT
1. Only paid requests shall be processed.
2. All certifications issued by PSA are considered rendered service. All requests submitted via PSA Serbilis and paid through the available payment channels will undergo processing under the Civil Registry System of PSA. Thus, these constitute rendered service, whether requests result to positive issuances or certifications (negative).
3. Returned mail delivery is also considered as rendered service. This includes wrong mailing address provided by requester, resident transferred to another address, and any other related circumstances.
4. For the issuance and release of Civil Registry documents: :
Only the following are authorized to receive birth certificates:
- The owner himself or through a duly authorized representative
- His/her spouse, parent, direct descendants, guardian, or institution legally-in-charge of him/her, if minor
- The court or proper public official whenever absolutely necessary in administrative, judicial or other proceedings to determine the identity of a person
- In case of the person's death, the nearest of kin
If you are the document owner and you are unable to receive the document, you may authorize a representative to do so on your behalf, but you will have to submit the following:
- Your signed authorization letter/Special Power of Attorney (SPA), stating the facts of the requested documents, number of copies, and purpose
- Your valid ID (with photo and signature), including PhilID
- The valid ID (with photo and signature) of your authorized representative, including PhilID
5. For delivery destinations within the Philippines:
For delivery destinations within Metro Manila, processing and delivery of paid requests take around three (3) to four (4) working days after receipt of payment; additional five (5) to seven (7) days for areas that are outside of courier's standard serviceable areas.
For delivery destinations in provinces, processing and delivery of paid requests take around six (6) to eight (8) working days after receipt of payment; additional five (5) to seven (7) days for areas that are outside of courier's standard serviceable areas.
6. For delivery addresses outside the Philippines, processing will immediately follow after payment, however, delivery will be dependent on the services of Philippine Postal Corporation (PHLPost). For special courier services through DHL Express, delivery charges shall be to the requester's/document owner's account and shall be billed separately and directly by DHL Express.
UNPAID REQUESTS
All PSA Serbilis requests that remain unpaid/waiting for payment for sixty (60) days or more will be discarded in the system.
